Signs you need an emergency wisdom tooth extraction today
Wisdom teeth arrive last, and the jaw often has no room for them. A tooth that can't come in straight gets stuck against the molar in front of it, or stays trapped under bone. Dentists call that impacted. A tooth that breaks partway through the gum leaves a flap of tissue over it, and food packs under that flap where no brush reaches. The gum swells, bleeds and tastes bad. That infection is the reason most people call us.

Other signs: a dull ache that radiates toward the ear, a jaw that's stiff when you open wide, a cheek that's puffy on one side. Flare-ups that fade and return mean the tooth will keep doing this until it's gone. Our wisdom tooth pain page walks through the causes.
If swelling has moved into the neck, if there's a fever, or if swallowing or breathing is hard, go to a hospital emergency room before anything else. Hospitals treat spreading infection and injuries; they generally don't treat the tooth itself. Phone us once you're discharged and we take it from there.
What happens at your same-day visit
Step one is a look at the area and a panoramic X-ray. The image shows which way the tooth is tilted, whether the roots are straight or hooked, and how near they sit to the nerve that runs through the lower jaw, or to the sinus floor above the upper teeth. That decides whether the tooth is a simple or a surgical extraction.
A simple extraction is for a wisdom tooth that has fully come through and sits upright. It's loosened and lifted out. A surgical extraction is for a tooth under the gum or bone, or one tilted sideways. The gum is opened, some bone may be taken away, and the tooth is usually split into pieces. Stitches may be placed. Both kinds are done here.

Frequently asked questions
How do I avoid a dry socket?
A dry socket is what happens when the clot in the empty socket comes loose and bone is exposed. For 72 hours after the extraction, no straws, no smoking or vaping, no hard spitting, and no swishing on the first day. Eat soft food and chew on the other side. The sign is pain that climbs on day three rather than fading; call us if that happens.
How long will I be sore?
Most people are sore for three or four days, with swelling that peaks around day two and then eases. A surgical extraction in the lower jaw tends to be the longest recovery. An over-the-counter pain reliever, used by the package directions, and cold on the cheek for the first day cover most of it.
Can an infected wisdom tooth be pulled the same day?
Often, yes. A mild gum flap infection doesn't stop an extraction. A large, hot swelling sometimes does, because the numbing doesn't take well in badly inflamed tissue. In that case we start antibiotics, drain what needs draining, and remove the tooth at a short follow-up. You'll know which situation you're in before you leave the chair.
